Summarize purpose of the contract, including why it was necessary: This contract is for COVID-19 vaccination conducted by Federally Qualified Health Centers (FQHCs), both within their clinics and in community settings, including shelters and other settings for people experiencing homelessness. The community-based vaccination work includes partnerships with MDH COVID Community Coordinators (CCCs) as well as
other community organizations.
This contract also included coordination of the COVID-19 vaccination work across the 17 Minnesota FQHCs, to streamline planning, funding, communication and technical assistance.

If this was a single source contract, explain why the agency determined there was only a single source for the services (if applicable) Federally Qualified Health Centers (FQHC) were uniquely effective in reaching Minnesota’s most vulnerable communities with COVID-19 vaccination and consistently vaccinated the highest proportion of patients in the first quartile on the CDC social vulnerability index.

The Minnesota Association of Community Health Centers (MNACHC) is the membership organization and coordinating entity for all of the 17 FQHCs operating in Minnesota. As such, MNACHC was uniquely positioned to coordinate COVID-19 vaccination work across FQHCs across the state and to streamline planning, funding, communication and technical assistance.
