Summarize purpose of the contract, including why it was necessary: 1. DHS (through Health Systems Coordinator) will execute contracts with existing Regional Resettlement Network Service (RNS) partners to fund family assister and community orientation services to address family wellbeing related to COVID-19, available to people within and outside Office of Refugee Resettlement eligibility parameters.
2. DHS (through Health Systems Coordinator) will maintain a contract with existing partners to provide the following Health Care Navigator services:
A. Maintain Helpline as gateway to COVID-19 testing and vaccination supports and resources to address other impacts of COVID-19 including:
i. Staff a statewide, public facing, multilingual help line Monday – Friday 9:00AM to 5:00PM.
ii. Provide immediate and ongoing health care systems navigation related to testing and vaccine access.
iii. Provide health education related to COVID-19 testing and vaccination.
iv. Connect to additional resources to meet identified non-vaccine related needs/impacts of COVID-19.

B. Provide expert TA and support to 25 community-based organizations statewide (Resettlement Network) on COVID testing and vaccine-related resources, access, and information:
i. Provide support to Resettlement Network related to COVID-19-specific resources.
ii. Provide regular updates on COVID-19 health information to Resettlement Network.
iii. Communicate back with state agencies related to gaps/barriers/needs for communities served.

C. Provide health navigation services to individuals impacted by COVID-19 including:
i. Immediate assistance connecting to testing, essential services and specialized supports.
ii. Ongoing health navigation for ongoing needs related to the impacts of COVID-19.
