Develop video success stories featuring community-driven approaches and successful strategies that help prevent diabetes and cardiovascular disease or improve the health of those living with cardiovascular disease and/or diabetes especially for communities experiencing the greatest health disparities. The videos will feature community partners and organizations who have had a positive impact in areas that align with State Plan objectives, such as community based non-profit organizations, Tribal partners, health care organizations, local public health, and other partners many others. Also develop supporting video communications products including a promotional video and photos to better engage multi-sector partners in the 2035 Diabetes and Cardiovascular Health State Plan. Videos will be shared through multiple communications channels by MDH and featured partners, including email and social media, and will be featured on MDH’s website and featured partner websites, as they desire.

The contractor completed the work within our expected timeline and was able to accommodate multiple community partner schedules.
This vendor brought exceptional story-telling skills to the project and created high quality videos.
The costs of this contractor were reasonable for the high-quality work provided.
Overall, community partners and MDH staff were very pleased with the work. This vendor is exceptionally easy to work with and brings a great deal of experience working on public health related issues that contributed to the success of the project.
