If this was a single source contract, explain why the agency determined there was only a single source for the services (if applicable) Conduent and its application MAVEN were initially selected by the state through a competitive process. This subsequent contract with Conduent was a single source because: (1) Enhancements and changes to the core application, which the State utilizes, must be completed by Conduent to stay compliant with their maintenance and support licenses. (2) MEDSS (the MAVEN application) is meeting the State's business needs and continues to be a long-term solution for the Department of Health.

Evaluate the contractor's timeliness As far as Change Requests go we have had multiple conversations with Conduent on what we both need to do better. Our Change Requests seem to be very slow in delivery and responsiveness to questions that we pose back to the developers.

Evaluate the contractor's quality Quality of work in the Change Requests can be improved. I think that getting the requirements done in a more complete fashion will help this.

Evaluate the contractor's cost The costs seem to be in line with other vendors.

Evaluate the contractor's overall performance I believe this could be improved upon and am looking forward to seeing what our new contract does for this.

Would you engage the contractor's services again? Yes

Describe any negative action taken during the duration of this contract (i.e., termination, pursuit of suspension, etc.): I do believe we have seen a lot of turnover with our team that takes care of our account from a sales, delivery, and development areas. This is natural with this type of work.
