Health Services resources are very limited in the area with no past interest of providing services to the DOC. Cook area Cook area Health Services is close geographically to the facility and the only one available in the area.

Evaluate the contractor's timeliness The contractor performed services in a timely manner, kept appointments and reached out for any scheduling issues and coordinated time off.

Evaluate the contractor's quality The value of this contract was excellent, the NP provided by Cook Area Health Services was an asset to Health Services provided at our Togo Facility.

Evaluate the contractor's cost The pricing of the contract was comparable with the community for a Nurse Practitioner.

Evaluate the contractor's overall performance MN DOC was extremely satisfied with the critical services provided under this Contract for incarcerated individuals at our Togo Facility. The contractor was easy to work with while we worked to hire a state employee for this role.
