Evaluate the contractor's timeliness Deliverables were often late. Next to no deliverables were completed on time and most timely work was driven by DHS staff oversight

Evaluate the contractor's quality The development, data collection, and follow-up did not meet the standard that DHS set for the work. The deliverables were below standard and often late. Deliverables were often less than anticipated and heavily worked on by DHS staff to meet necessary standards

Evaluate the contractor's overall performance The survey was completed generally on time. Though the outcome of the survey, data, was lacking based on the criteria DHS set.

Describe any negative action taken during the duration of this contract (i.e., termination, pursuit of suspension, etc.): DHS terminated the contract due to other financial needs as well as determination that internal staff could more easily complete this work with less effort. At times DHS also withheld payment for work that was billed but not completed.
