Summarize purpose of the contract, including why it was necessary: Contract to continue the use of the EHR which includes product licensing, cloud/hosting, professional services and various functions implemented over time that allow for the storage, retrieval and use of clinical information for the purpose of treating over 10K patients under the direction of DCT psychiatrist, psychologists, nurses, social workers and other professionals in a health care setting. Amendments have included new functionality, expansion of support, additional licensing, product maintenance, etc. Specific project/product implementation includes Computrition (nutrition program), UAT expansion, CareManager, Fail over testing, ICD-10. This contract complements the M&O contract with same vendor PTK-149639. System is Priority 1 Health & Safety application and must be maintained without break 24/7/365.

If this was a single source contract, explain why the agency determined there was only a single source for the services (if applicable) This is an existing Electronic Health Record system already in place for DHS which has been functioning for nearly 20 years. No other product meets the specific need of the DCT administrations. New contract will re-establish the single source justification.
