Summarize purpose of the contract, including why it was necessary: Independent Lifestyles, Inc. will collaborate with MDH, local public health, and other partners to help central MN community members with disabilities navigate systems and resources for their COVID-19 related needs; provide planning and support for testing and vaccination, provide mental health support, and work in close communication and collaboration with DEED, MDH and other partners throughout the project. Independent Lifestyles, Inc.’s work as a CCC was necessary to serve the community of people with disabilities in Central Minnesota in response to the disproportionately negative impacts they bore from the COVID-19 pandemic. According to data from the CDC, people with disabilities are 50% more likely to be hospitalized from COVID-19 and more likely that people without disabilities to experience barriers in getting COVID-19 vaccine and boosters. The National Council on Disability also reported that social isolation resulting from the pandemic has increased national rates of reported metal health disabilities and adversely affected people who already experienced higher rates of social isolation, including older adults and people with disabilities.
