contractor did not submit required documents on time. The workplan/budget were submitted 3 weeks late. No required invoices or reports were submitted during the contract period or following subsequent months despite a long process of trying to support them to get documents submitted.
The contractor provided a few work deliverable examples during the contract period. The examples indicated quality work related to the workplan. How ever they did not provide the required document to verify work was completed. The contractor did attend monthly check-in as and the required trainings.
Cost of work deliverables provided in the workplan/budget were reasonable. However no invoices were submitted.
The contractor was unable to provide documentation of work deliverables. MDH provided a number of deadline extensions and offered additional meetings to support.
The contractor cited health issues related to COVID as a barrier to submitting required invoice and deliverable reporting documents.

Closing this contract after a long exhaustive process of trying to get the vendor to submit reports/invoices. We were not successful in connecting with them to verify if any of the deliverables in his work plan/budget were completed.
