Personnel listed on original QMP and proposal were not performing or checking work on 60% set.
90% submittal did not contain calculations until requested.
30% and 60% beam designs did not follow MnDOT standards. 60% abutment drawings did not follow MnDOT standards. Errors in quantities, a few dimensions, WP references, bar lengths, etc. were still noted after the 95% review set. Contractor did work to eventually include comments and correct errors, but with much guidance from MnDOT.
