Evaluate the contractor's timeliness Throughout the contract period, Forma ACS executed all deliverables either on-time or early. The contractor was able to successfully adjust timelines when deadlines were moved up.

Evaluate the contractor's quality Forma ACS provided the highest quality work in a timely and transparent manner. Where there was need for refinement on work products, the contractor was engaged, receptive and flexible, and ensured that final deliverables were on-time.

Evaluate the contractor's cost Forma ACS's costs were reasonable and in-line with actuarial market. The contractor provided backing information on all hours billed, ensuring that DHS only paid for work that was required and requested by DHS.

Evaluate the contractor's overall performance Excellent. Forma ACS played an integral part in the development and initial implementation of the IHP pilot project.
