Contractor provides services, information, and innovations to achieve our mission of assisting individuals to age well and developing the capacity of communities to care for an aging population. Contractor is the designated Area Agency on Aging for the seven-county metro area, including Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties. Many of the initiatives and activities of Trellis, including the Juniper health promotion programs, have a broader geographic reach across the state of Minnesota.
As an Area Agency on Aging, Contractor ensures services and policies are tailored to the needs and aspirations of the people living in local communities. Contractor awards federal Older Americans Act funding to organizations that provide services to older adults and caregivers and provide direct services to older adults and caregivers as part of the statewide Senior LinkAge Line®. Contractor is a leader and innovator, integrating health and social care to produce a better quality of life and health outcomes — and doing so in a way that delivers crucial services while containing escalating costs.
Juniper® (yourjuniper.org) is a program of Trellis. Juniper uses a network approach to disseminate evidence-based health and wellness classes to Minnesotans across the state to help them improve their health and reduce disease and injury. Juniper contracts with community-based and healthcare organizations to offer education led by Juniper-certified class leaders.
Juniper program participants can easily find and register for evidence-based health promotion classes, including the National DPP, using the Juniper website at yourjuniper.org. This sophisticated management information system (MIS) allows community based National DPP providers to schedule and manage classes and report participant self-reported health outcomes. The MIS makes creating financial relationships with third-party payers seamless and scalable.

The contractor’s work was consistently of high quality. This contractor had substantial expertise and commitment to successfully enroll populations at greatest risk into the DPP; to test and expand a Minnesota umbrella hub organization payment model; and design and pilot an MCO member recruitment and enrollment system for Minnesota National DPPs.
The contractor’s costs were reasonable.
This contractor’s overall performance was excellent and resulted in the achievement of many goals related to the objectives set out within the contract. Trellis is doing important work and with their umbrella hub arrangement and their evidence-based health and wellness classes. These initiatives help Minnesotans improve their health and reduce disease and injury.
